Submit Application

Complete our online application form with information about your family and your child's educational background. For Elementary and above, a brief student assessment is included.

Acceptance & Enrollment

Upon acceptance, our team guides you through enrollment, tuition, and financial aid, ensuring every family can access the MHA-CYOS experience.

Application Requirements

Early Childhood (Pre-K – K)

Complete the Early Childhood Application. Submit health and immunization records. New kindergartners must provide updated immunization documentation by the first day of school.

Grades 1–8

Complete the Grades 1–8 Application. Provide transcripts and standardized test scores from previous schools. A brief student assessment may be included. Application fee: $110 per child (max $220 per family, non-refundable).

Grades 9–12

Complete the Grades 9–12 Application. Students may need Math and English placement exams. A school visit or Zoom interview is strongly encouraged. Dorm students must also submit a Medical Authorization Form and insurance card copy.

MAKING MHA ACCESSIBLE

FINANCIAL AID

MHA-CYOS is committed to making Jewish day school education accessible to all eligible families. We work with families to identify financial aid options and tuition assistance programs that can make enrollment possible.

Financial aid applications are managed through the FACTS Financial Aid platform, a confidential, streamlined process for families to apply for need-based assistance. We encourage all families who need assistance to apply early.

How do I apply for financial aid?

Families must submit a FACTS application. Access the portal through the link below. Incomplete submissions will not be processed.

What documents do I need?

Your most recent tax return, W-2(s) and/or 1099(s), and your prior year tax return. All documents must be uploaded directly to the FACTS portal.

What happens after I submit?

Your information is sent to a third-party administrator for determination. Your financial aid award will appear on your tuition agreement.

Can I appeal my determination?

Yes. Appeals are available if your circumstances have changed. Submit relevant documentation not previously provided; independent third parties review all appeals. Contact us for the current appeal deadline.
